NAME, ADDRESS, AND OTHER INFORMATION
Please print or type clearly and provide all information as requested. Your badge(s) will be printed from this information. Indicate if you wish your email address be withheld from exhibitors in this section. Degree information will be taken from this section for your badge. Please indicate the order in which you wish degrees to be displayed.
Americans with Disabilities Act
If you have special needs under the Americans with Disabilities Act, please check the box in step 1 of the registration form. We will call you to discuss your particular needs.
Indicate special dietary needs in this section. Selections are for meals served at Postgraduate Courses, Meet the Professor Seminars and Workshops only.
NATIONAL PROVIDER IDENTIFIER
In order to comply with recently enacted federal law, the American Thoracic Society will start collecting the National Provider Identifier (NPI) number of U.S. physicians (MDs and DOs) who attend the ATS International Conference. Attendee NPI numbers will be shared with healthcare convention marketers and exhibitors to assist them in complying with federal disclosure laws on public reporting of “transfers of value” between industry and physicians. Providing your NPI number to the ATS does not necessarily mean that you will be publicly reported. If you need assistance in locating your NPI number click here.
This is an important section for the ATS. The information collected in this section will help the Society know who is attending the conference and will be used to plan the sessions for future conferences.
GENERAL REGISTRATION FEE
REGISTRATION FEES UNCHANGED FOR THREE CONSECUTIVE YEARS!
Registration fees vary according to your membership or non-membership category and the date by which you register.
Full Conference Registrants
The general registration fee, offered at a reduced rate to members and non-members who register on or before March 12, allows you to attend over 90 percent of the meeting events without additional fees or tickets. It also entitles you to attend social events and the Exhibit Hall. Non-member post-doctoral trainees and students from any discipline must provide information about their training program as indicated in order to obtain the much reduced student conference registration rates.
ATS members receive a discount when registering. To become a member, click here. Upon completing the online membership application, a username and password will be emailed to you. Sixty minutes after receipt of this email, you will be able to register for the Conference as a member and receive the discounted registration rate. Full members receive a larger discount than affiliate members. Non-members receive no discount.
Research Assistants/Administrators/Association Executives
This category is provided for those individuals who attend for reasons other than obtaining CME, or who are not healthcare professionals working clinically in pulmonary, critical care or sleep medicine. For instance, research assistants, research administrators, research coordinators, executive staff of related professional organizations, or other individuals with an organizational but not clinical interest in the ATS Conference would register under this category. Registrants in this category may attend sessions that do not require additional registration, but may not register for postgraduate courses, seminars, workshops or Assembly dinners/receptions. Registrants in this category are not eligible to receive continuing medical education credits. If CME or other professional recognition is desired, then a member or non-member registration is required.
Registrants in this category must accompany a full Conference registrant and may attend only the Exhibit Hall, special lectures and open receptions. Those registering in this category are not eligible to receive continuing medical education credits or attend postgraduate courses, seminars or workshops. Spouses/partners/guests who do not wish to attend the Exhibit Hall, special lectures, or open receptions, do not need to register or pay the guest fee.
The ATS offers a Weekend Special rate for those unable to attend the entire conference. This registration category will include one postgraduate course on Saturday, May 18 and a one-day only registration for all the sessions held on Sunday, May 19. Click here for Weekend Special fees.
Members Of The Press
The ATS offers a full press room at the International Conference. All members of the press must be approved by the ATS Communications Unit before registering. For more information about how to register, scheduled press conferences and press releases on research to be presented at ATS 2013, please contact Nathaniel Dunford at firstname.lastname@example.org.
